Registering with the Practice
Patients wishing to join the Practice will be asked to complete a registration form and health questionnaire. You will then be asked to make an appointment with the Healthcare Assistant for a routine health check.
If you are on regular medication, please bring an up to date list along to this appointment.
Please note that our practice has a policy with respect to Benzodiazepines and Z drugs (such as Diazepam, Temazepam, Zopiclone etc). All patients applying to join the list need to understand that if you are on any of the above medications you will be asked to undertake a reduction programme after which the items will no longer be prescribed, this is in line with local prescribing guidelines.
